When Vit Spěvák jumped out of a helicopter, his parachute was released prematurely for unknown reasons, getting caught behind the skids of a police helicopter. Flight instructor Jiří Pešek immediately tried to get Vít Spěvák back into the machine, but his attempt to rescue his colleague was interrupted by a reserve parachute, which also opened. Unlike the main parachute, however, it did not hook behind the skids, but was sucked in and wound up by the helicopter's tail rotor carrying the superintendent, who died instantly.
